Diabetes Education Program
Where:
Community Medical Center Healthcare System 3rd floor in the diabetic classroom
When:
Monday evenings 6:15 PM to 9 PM, Wednesday mornings 9:15 AM to 12 PM (Additional classes are available as needed)
Why:
Learn to live a healthier life while coping with diabetes
Who:
Adults who have diabetes
Your Requirements:
1. Obtain a written referral and/or prescription from your primary care physician.
2. Call the Nutrition Department at (570) 969-7272 to register and to schedule your initial assessment.
3. Attend all group sessions.
4. Attend your scheduled counseling sessions.
CMC will notify your primary care physician when you have finished the program.
Most insurance plans are accepted.

Individual Sessions: · Initial Assessment · Follow-up session based on individual needs
Four Group Sessions to include the following topics: · Overview of diabetes · Setting goals and coping with diabetes · Nutrition and meal planning · Medication ad monitoring · Exercise · Pattern management · Prevention, direction and treatment of acute and chronic conditions · The importance of hygiene and foot care · Benefits of management · Building a support system with healthcare professionals, community members and family members
