POSITION SUMMARY

Provides medical social work services, including counseling and discharge planning services, to patients/families and significant others to improve their psychosocial functioning while enhancing proper utilization of available health and welfare services. Provides a leadership role to the discharge planning component of patient care serving as a resource for the identification and implementation of post hospital care and service delivery systems.

QUALIFICATIONS

Education: Masters degree in Social Work required.

Experience: Minimum of two (2) years of supervised clinical experience in a medical setting is preferred.

Specific Requirements: Must be able to speak, write and understand the English language to effectively communicate with predominantly English-speaking population. High tolerance for stress. Must be able to effectively deal with crisis, loss and hostile situations. Must possess sufficient maturity and balance to address the often difficult and sordid conditions inherent in position.
